Qualified Social Worker -Norfolk - Hospital Care Management Team
Experienced Qualified Social Worker needed to work in a Hospital Care Management team. You will need to have experience working in a hospital environment and have excellent communication skills. Duties will include preliminary assessments, working with patients and those professionals required in a resultant care plan, organising safe and effective arrangements for care when leaving hospital and provide general information on how and where to apply welfare benefits. This is a temporary interim position with the possibility of being extended at the end of its tenure. This is a particularly challenging role in which the right candidate will be working principally independently.

You must have a DipSW or a Degree in Social Care, a current CRB and be registered with the GSCC.
